INSURANCE

Please note that as a one-on-one concierge health and wellness clinic providing personalized treatment plans, we are out-of-network for insurance providers. This allows your therapist to offer the services you need, when you need them, without the constraints of insurance. Payment is collected at the time of service (cash, credit card, check, flexible spending accounts, and health savings accounts are accepted). If you are interested in understanding your potential level of reimbursement for our services, please contact your insurance provider and inquire about coverage for "out-of-network physical therapy" and any applicable deductibles.

 

If you would like a super bill, please request one at checkout, and we will email you an itemized bill to independently submit to your insurance company for reimbursement.

CANCELLATION & MISSED APPOINTMENT POLICY

If you are unable to keep an appointment, please contact us as soon as possible. We request that cancellations or rescheduling be done at least 24 hours prior to your scheduled visit to allow other patients the opportunity to use that time. Appointments cancelled with less than 24 hours' notice, or not cancelled at all, will incur a $100 fee due to scheduling inconveniences.

 

You may cancel your appointment via email, phone call, or text, as long as the message is sent 24 hours prior to the appointment start time. In the event of a true medical emergency, the cancellation fee will be waived.

REFERRALS

In the state of California, physical therapists have direct access, which means you DO NOT need a physician’s referral to receive a physical therapy evaluation or to get started.

REFUND & RETURN POLICY

All sales on services and packages are final, including discounted service packages. Purchased packages and discounted treatment plans are non-refundable, and visits on account must be used within one year of purchase. Any unused visits at the end of your therapy journey can be gifted or saved for future use.

YOUR FIRST VISIT

At Hybrid Performance Method, we use Jane App for all forms and payments. After scheduling an appointment, you will automatically receive forms to complete before your first visit. These intake and consent forms are required prior to your initial session.

 

The first session is an evaluation, during which your therapist will take a subjective report of your symptoms and discuss various topics, including the history of your symptoms, your past medical history, and any surgeries you have had. Your therapist will then guide you through a basic movement screen to observe your movement patterns.

 

Additionally, there will likely be an orthopedic exam to assess the strength, range of motion, and flexibility of your neck, shoulders, trunk, hips, and legs. Video footage of your swimming stroke may be requested during the initial evaluation to identify technique abnormalities.

These treatments will include a variety of techniques, including:

  • Manual therapy and trigger point release

  • Cupping

  • Connective tissue manipulation

  • Neuromuscular re-education exercises for proper muscle activation and engagement

  • Self-care techniques to help with pain and function

  • Education regarding pain management, breathing, posture, activity modifications, and recovery strategies

  • Exercise prescription to target stability and mobility of the region of pain as well as the body as a single unit
